I, —, the owner of the policy referenced above, authorize my carrier to release any and all in-force illustrations, policy values, contractual information, rider details, beneficiary designations, and related records pertaining to that policy directly to GetSure Insurance Agency (290 Via Casitas #102, Greenbrae, CA 94904). Requests made by GetSure on my behalf may be submitted by mail, email, fax, or telephone, and any faxed or emailed copy of this authorization shall be considered as valid as the original.
This authorization remains in effect for twelve (12) months from the date of signing, or until withdrawn in writing. This is not a replacement request and does not designate a new agent of record.
By signing electronically below, I execute this authorization under the federal ESIGN Act and applicable state equivalents; my electronic signature has the same legal effect as a handwritten signature on paper.
